(2019) This essential book is now available from a bunch of POD publisjhers and Gyan in India, but I haven't seen how well any of them have produced it.
This PDF is very nicely OCR'd and doesn't have all the typos and errors that most do. The photos in the original are absent of course, as well as other oddities like the contents of the Tables at the end, etc. The page numbering may not correspond to the TOC or Index.
But the original of this is a >500 page book and sells for like $200 if it ever appears, so this is a great stopgap resource.
